You have two ways to sign the waiver

1.  click here for-->  Online Waiver  Read and check the box and a confirmed copy of this will be emailed back to you.  

           Or

2. Print the Printable Waiver below out , sign and mail or scan and Email to below address.   This needs to be done yearly.
